Appointments
We see clients primarily through appointments that can be made by calling (254) 519-4162. Regular office hours are Monday -Thu, 8:00 a.m. to 7:00 p.m. and Friday 8:00 a.m. to 2:00 p.m. After hours appointments may be available with some providers.
Medications
Medications may be prescribed for your child as a part of treatment. Be sure to discuss refills with the physician during the appointment. To receive a prescription refill without delay, the physicians ask that you call our clinic staff at least seven days in advance. Online Refill Request

New Patient Packet - Please call the clinic first to schedule an appointment, then print and complete the informataion requested in the pdf form, and bring it with you to your appointment.
Release of Information Request - Please print and complete all the information requested in the pdf, and either bring or fax it to the clinic. Our fax number is (254) 519-3464.

Cedar Crest Clinic Policies
- Cedar Crest respects the right of privacy of all our clients. Confidentiality is observed and protected at all times.
- We require at least 24-hours notice if you will be unable to keep your appointment. If you do not give us 24-hour notice, a fee of $50 may be charged to your account.
- Copayments, deductibles, and any existing balances are due at the time of the appointment.
- If you request the doctor or therapist to complete forms, you will be charged a fee of $25.00.
- If you request copies of your child's medical record, you will be charged a fee of $10.00 for the first 10 pages, plus 15 cents per page thereafter to a maximum of $25.00.
- If a check you write us is returned, a fee of $35.00 will be charged to your account.
Emergencies
If you feel your child is in danger of harming himself or someone else, please call 911 or go to the nearest Emergency Room. If you need to contact our physician on call, please call Cedar Crest Hospital at (254) 939-2100.
